8. Where we get personal information from

We obtain your details when you enquire about our care and service, when you join the practice, when you subscribe to our newsletter or register online, when you complete a registration or medical history form, from family members or carers, and when another healthcare provider refers you for treatment at our practice.


9. How long we keep information

We minimise the data that we keep, and do not keep it for longer than necessary.

We keep your records for 11 years after the date of your last visit to the Practice or until you reach the age of 25, whichever is longer. At your request, we will delete non-essential information (for example, some contact details) before the end of this period.


10. How we store information

We store your personal information securely on our practice computer system. Only those working at the practice have access to your information. They understand their legal responsibility to maintain confidentiality and follow practice procedures to ensure this.


We take precautions to ensure the security of the practice premises, the practice filing systems and computers.


We use high-quality specialist dental software to record and use your personal information safely and effectively. Our computer system has a secure audit trail, and we back up information routinely.

platform provider on our behalf, and key details are added to our customer relationship

management (CRM) system.

• Recipients (categories): Chat platform provider (hosting/storing chat records),

integration/automation provider (routing enquiries between systems), and CRM

provider (storing enquiry and contact details), all acting as our processors on our

instructions.

• Retention: We keep chat records only for as long as needed to handle your enquiry

and to ensure our systems are running smoothly. Enquiry and lead records remain

in our CRM until you ask us to remove them, unless we are required to keep them

longer by law. If we stop using our chat provider, we will ensure any copies they hold

are deleted or returned to us within 30 days.

If your concern relates to the Practice Manager/Registered Manager, you can speak to any member of our team, who will ensure it is passed to the appropriate person. Alternative arrangements are in place to ensure your concern is handled fairly.

If you prefer, you can ask someone to raise a complaint on your behalf, such as a family member or representative.


What happens next?


We will acknowledge your concern

We aim to acknowledge your complaint as soon as possible, and not later than 3 working days after we receive it.

At this stage, an offer to discuss your complaint will be provided. This is a chance for you to understand:

• How your complaint will be handled

• The investigation period

• When a response can be expected

You may choose whether to have this discussion or decline it.


Early Resolution

Where possible, we aim to resolve your concern quickly.


Investigation

If your concern is more complex or cannot be resolved straight away, we will carry out a full investigation.

General Requirements


All patients are required to pay a deposit before an appointment is booked.


Deposits must be clearly recorded on the system under Deposits, including:

  • Clinician initials

  • Staff member who took the booking

  • Deposit amount


Routine Appointments

Routine appointments (e.g. examination and hygiene) require a 50% deposit at the time of booking.

Examples:

  • Examination £55 → Deposit £27.50

  • Hygiene £90 → Deposit £45

The remaining balance is payable at the appointment.

If the patient books a further appointment, a new deposit must be taken at that time.


Clinician-Specific Deposits

  • Muataz: £50 consultation deposit
    Treatment costs and further deposits will be advised by the clinician.

  • Reem: £50 consultation deposit
    Treatment costs and further deposits will be advised by the clinician.


Treatment Appointments

Treatment deposits are based on the time booked in the diary:

  • 30 minutes → £50 deposit

  • 60 minutes → £100 deposit

  • 90 minutes → £150 deposit

Note: A higher deposit may be required if instructed by the clinician, depending on the treatment plan.


Account Handling


Deposits or credits must not be transferred between different clinicians or applied to treatment provided by another clinician.


Staff should review and understand the patient’s account before the patient reaches reception, ensuring confidence and efficiency when taking payments.


All relevant treatment plans must be checked and signed where applicable.


If you are unsure how to manage accounts or payments, please request additional training from the Practice Manager.

Cancellation Policy

Cancellation Policy

At Chester Cosmetic & Dental Care, we are committed to providing excellent care and ensuring our patients have timely access to appointments.


If you need to reschedule your appointment, we kindly ask that you give us as much notice as possible.


This allows us to offer the appointment to another patient and maintain an efficient schedule.


Our cancellation policy is as follows:

• 72 hours’ notice or more:
We will be happy to reschedule your appointment to another suitable time at no charge.

• 48 hours’ notice:
We kindly request payment of the remaining balance for any upcoming treatment in order to secure your next appointment.

• 24 hours’ notice or less:
Late cancellation or missed appointment fees will apply.


Please note that if two appointments are missed or cancelled late during a course of treatment, we may need to review your treatment plan.
